Cost

The total cost of the USCIS Immigration Medical Exam is $695 per person, regardless of age.

A non-refundable $350 deposit is required at the time of scheduling and is applied toward the total cost of your exam.

The remaining balance of $345 is due at your physician appointment.

Additional costs may apply if:

  • Vaccinations are needed
  • A chest X-ray is required following a positive tuberculosis screening test
  • Additional testing is required by USCIS guidelines

How the Process Works

Step 1: Enrollment & Scheduling

Before scheduling, all patients must:

  • Complete enrollment with DawnMD
  • Have a payment method on file
  • Pay the required deposit

Once these steps are completed, our team will schedule your appointments.

Step 2: Nurse Visit

During your visit with Nurse Jaimie, we will:

  • Review your immigration paperwork
  • Review your vaccination records
  • Determine whether any vaccines are required
  • Obtain bloodwork and USCIS-required laboratory testing
  • Discuss any additional records or next steps needed

Step 3: Physician Visit

You will meet with Dr. Shaleah Jones, USCIS Civil Surgeon, for your immigration medical examination.

During this visit:

  • Your medical history will be reviewed
  • Your physical examination will be completed
  • USCIS-required documentation will be finalized

Step 4: Final I-693 Packet

Once all testing and requirements have been completed, your Form I-693 will be prepared.

You will receive:

  • A copy for your records
  • A sealed USCIS packet for submission

Please do not open the sealed USCIS packet.


Required Documents

Please bring:

  • Government-issued photo identification (such as a driver's license or state-issued ID)
  • Passport (required if you do not have a state-issued photo ID)
  • A-Number (if applicable)
  • Vaccination records
  • Medical records from healthcare providers you have seen in the United States
  • The two forms Nurse Jaimie will send you:
    • USCIS Medical Exam Screening Checklist
    • Authorization to Release Health Records

If records are not in English, certified translations may be required.

Vaccination Records

Providing vaccination records before your appointment helps us determine which vaccines may be needed and can prevent unnecessary vaccinations, costs, and delays.

Common vaccine records include:

  • Hepatitis A
  • Hepatitis B
  • MMR (Measles, Mumps, Rubella)
  • Varicella (Chickenpox)
  • Tdap/Td
  • Polio
  • Influenza
  • COVID-19 (if applicable under current USCIS requirements)

Vaccination requirements are determined by USCIS guidelines and vary based on age and individual circumstances.


Tuberculosis (TB) Screening

USCIS requires tuberculosis screening for all applicants, ages 2 and over.

If your TB blood test is positive, additional evaluation may be required, including:

  • A chest X-ray
  • Additional testing if indicated

A positive TB screening test does not automatically prevent completion of your immigration medical exam. Our team will guide you through any additional requirements.


Frequently Asked Questions

How long does the process take?

Most cases are completed within approximately one week, depending on laboratory results, vaccination requirements, record availability, and whether additional tuberculosis evaluation is required.

Our USCIS appointments are generally scheduled on Thursdays, allowing us to efficiently coordinate your nurse visit, physician visit, and final paperwork.

Can I complete everything in one visit?

No. USCIS medical exams at DawnMD require two appointments:

  1. A nurse visit for record review, vaccination review, and laboratory testing
  2. A physician visit with Dr. Shaleah Jones, USCIS Civil Surgeon

This process helps ensure all required testing and documentation are completed accurately before your final paperwork is prepared.

Do I need my vaccination records?

Yes. Providing vaccination records is strongly encouraged and can help prevent unnecessary vaccinations, costs, and delays.

Can DawnMD obtain my records?

Yes. We can provide a medical records release form to help obtain records from previous healthcare providers.

Do I need an interpreter?

You do not need an interpreter if you are fluent in English. If you are not fluent in English you must provide your own interpreter for your visits. DawnMD does not provide any interpreter services.

Is my appointment reserved when I submit a request?

No. Appointment requests are reviewed by our team. Your appointment is not reserved until enrollment, deposit requirements, and scheduling have been completed with our office.
